Home From The Hospital is an assessment and advice service that evaluates your needs and helps you contact your insurance company, your physician, your pharmacy, community agencies, equipment providers and family and friends to get your needs met.

Home From The Hospital is a one-time fee for a one-time visit to your home on the day you get out of the hospital or as you direct. Follow-up calls to the HFH Nurse are free to you for a one-week period, after the initial visit, to get your questions answered and to meet your follow-up needs.

Home from the Hospital Staff are experienced health professionals and medical discharge and wellness counselors, who are used to working with physicians, home health agencies, equipment providers and pharmacy plans.

Home From The Hospital Associates are in your home with their medical background and experience to help you to

  • Understand your Discharge Instructions from your Doctor
  • Make sure you have the correct medications and are taking them correctly
  • Make sure your equipment, home health service and safety needs are met
  • Assist with needed contacts with your Physician,Home Health Agency, Equipment provider and Insurance company
  • Make sure you have appropriate follow-up visits scheduled with your Doctor
  • Make sure you have appropriate help at home from family, friends or others

We make sure you have just the help you need to stay well and be well at home.

Home From The Hospital is a voluntary, self-pay service that is not covered by any health insurance. You make the payment at or before the time of the service.
